Request to Amend/Supplement Contingent Fee Application Form – This Form should be used for amendments other than changes to Contact Counsel or Fee Interest Counsel. A single form may be used for any/all settlements, but the unique Rubris Crosslink ID must be provided for each Application to which the form applies.

Contact Counsel Change Form – UPDATED – This Form should be used to identify new Contact Counsel to be substituted for current Contact Counsel.

Changes to Contact Counsel must be approved by the Fee Panel. Once approved, Contact Counsel and all Fee Interest Counsel are required to resubmit Certifications. Clients will also be notified of changes to Counsel.

Fee Interest Counsel Change Form – UPDATED – This Form should be used to change the Fee Interest Counsel identified in a submitted application.

Changes to Fee Interest Counsel must be approved by the Fee Panel. Once approved, Contact Counsel and all Fee Interest Counsel are required to resubmit Certifications. Clients will also be notified of changes to Counsel.
